My Local Dentists West Ryde – Patient Referral Program – Terms and Conditions

1. Eligibility

• The Program is open to existing patients of My Local Dentists West Ryde who are 18 years of age or older.
• Referrals must be new patients who have never visited My Local Dentists West Ryde before.

2. How It Works

• Existing patients (“Referrers”) can refer friends or family by having them using a referral card available at reception.
• The referred person (“Referee”) must schedule and attend a new patient appointment for the referral to qualify.

3. Referral Rewards

• Referred patients will receive 10% off their dental treatment, exclusions apply, valid for one treatment only.
• Rewards cannot be exchanged for cash or transferred to another person.

4. Limitations and Exclusions

• Referrals of immediate family members living in the same household may not qualify.
• Referrals must be made before the new patient’s first appointment is booked.
• 10% off discount applies to out-of-pocket costs and cannot be combined with insurance-covered services or other promotions

5. Fraud and Misuse

• My Local Dentists West Ryde reserves the right to disqualify any participant who attempts to game or abuse the referral program.
• Suspected fraud or misuse may result in termination from the Program and cancellation of any pending rewards.

6. Program Changes

• My Local Dentists West Ryde reserves the right to cancel or modify the Referral Program or these Terms at any time, without prior notice.
• Any changes will be effective immediately upon posting on our website or at the clinic.

7. Legal Disclaimer

• The Referral Program complies with applicable state and federal healthcare marketing laws.
• No part of this Program is intended to induce or reward patient referrals where prohibited by law.

My Local Dentists Leichhardt Mother’s Day Special Offer Terms and Conditions

1. Offer Inclusions

This $450 package includes:

• Comprehensive dental examination
• Scale and clean
• Fluoride treatment
• Take-home teeth whitening kit (custom trays and whitening gel)

2. Eligibility

• Offer is valid for new and existing patients aged 18 and over.

3. Booking & Availability

• Offer must be mentioned when booking the appointment.
• Appointments are subject to availability.
• Valid until 31st of May 2025 or while stock lasts.

4. Limitations

• Not redeemable for cash or transferable to other services.
• Cannot be combined with any other discounts or offers.

5. Health Fund Use

• If you have private health insurance, you may still claim eligible components (e.g., exam, clean) depending on your level of cover. The remaining gap payment will apply to reach the $450 package value.

6. General

• My Local Dentists reserves the right to modify or withdraw the offer at any time.
• Final suitability for whitening is determined by the treating dentist.

Gap-Free Fissure Seals Terms and Conditions

• Offer is available until 31st of May 2025
• Gap-Free Fissure Seals can only be redeemed if the patient has a Check-up and Clean appointment either on the day or at My Local Dentists in the last 6 months
• Check-up and Clean and X-rays provided on weekends may have an out of pocket gap payment, check individual locations for more information
• Fissure Seals, General Check-up, Scale and Clean, Intraoral x-rays, OPG’s are gap free, provided that the health fund pays money towards the treatment (i.e 161 011 012 114 111 121 072 022 037 item numbers)
• It is the responsibility of the patient to check if your health cover still has dental funds
• Health Fund cards must be present on day of treatment or full price will have to be paid and patient will have to claim for themselves

Gap-Free Mouthguard Promotion Terms and Conditions

• Offer is available until 31st of March 2025
• Limit of 1 Gap-Free Mouthguard per person
• Gap-Free Mouthguard can only be redeemed if the patient has a Check-up and Clean appointment at My Local Dentists in the last 6 months 
• Gap-Free mouthguard will be a 3mm acrylic custom mouthguard
• Check-up and Clean and X-rays provided on weekends may have an out of pocket gap payment, check individual locations for more information
• Mouthguard, General Check-up, Scale and Clean, Intraoral x-rays, OPG’s are gap free, provided that the health fund pays money towards the treatment (i.e 151 011 012 114 111 121 072 022 037 item numbers)
• It is the responsibility of the patient to check if your health cover still has dental funds
• Health Fund cards must be present on day of treatment or full price will have to be paid and patient will have to claim for themselves

NO GAP Check up and Clean Terms

• Gap Free promotion only applies on weekdays
• Check up and clean and x-rays provided on weekends may have an out of pocket gap payment, check individual locations for more information
• General Check-up, Scale and Clean, Intraoral x-rays, OPG’s, Lateral Cephs are gap free, provided that the health fund pays money towards the treatment (i.e 011 012 114 111 121 072 022 037 item numbers)
• It is the responsibility of the patient to check if your health cover still has dental funds
• The Scale and Clean does not include a second visit for further treatment, the use of anaesthesia, deep cleans, periodontal charting
• This offer is available once in a six month period.
• Health Fund cards must be present on day of treatment or full price will have to be paid and patient will have to claim for themselves
